Documentation Guidelines
Any third-party documentation should be from a licensed professional, and should demonstrate a current impairment that substantially limits a major life activity, and the need for requested accommodations. Previous IEP or 504 plans can also accompany your application.
Third-party documentation is helpful, although not always required. We may be able to proceed with an intake or may implement conditional accommodations while waiting for documentation. More extensive documentation may be needed for graduate school.

Student Life Accomodations
Please fill out the housing application if you are requesting a student life accommodation. Student life accommodations must be renewed each semester through the same housing application.
Housing accommodations are only a priority for the requested housing, and not a guarantee. Housing is based upon availability, and you must still complete the Housing Office’s related forms.
Priority single/double occupancy
Priority to only have one roommate in a triple room, or no roommate in a double room
ADA rooms
Assignment of a dorm room and bathroom that are ADA accessible
Off-campus housing
Priority to live off campus, or in Ocean View apartments
Please fill out the housing application if you are requesting a student life accommodation. Student life accommodations must be renewed each semester through the same housing application.
1. Review the Food Allergies page on the Westmont Dining website.
2. Request a Meal Plan Modification through applying with the ARO. Once we receive supporting documentation, we will invite you for an intake appointment.
3. If you qualify for a meal plan modification, we will connect you with Bon Appetit's dietitian. In your meeting, the Nutrition Manager or RDN will work with you to determine dietary options and create an Individual Medical Meal Plan (IMMP).
5. If Dining Services determines that your dietary needs cannot be met through an Individual Medical Meal Plan (IMMP), the ARO will be notified that a formal exemption may be needed. An exemption will not be considered until all previous steps have been completed.

Priority parking pass
Parking accommodations are only a priority for a permit, and not a guarantee. Parking is based upon availability, and you must still complete the Housing Office’s related forms.
Golf cart access
Golf carts are rented out by the physical plant. You can request one through the Student Health Center, or we can request one on your behalf if you register with the ARO.

ESA
Review and complete Westmont’s Assistance Animal Policy.
Fill out the ESA Questionnaire and provide copies of all vaccinations (if applicable), health records, certification of spay/neuter, certification of health, and license number.
Email a photo of your ESA in its crate or home to aro@westmont.edu.
Have your provider complete and return will need to complete and return the following two forms to the ARO: Psychological Disorder Documentation Form ESA Provider Form
Service Animal

2. Complete the Intent to Use a Service Animal form.
